Nomination Forms

Each nominee should fill out the award form for which they are nominated.  Except for the first page, all the information will be used in the judging process.  In addition to the nomination form, each nominee should submit a one paragraph biographical sketch of themselves to be used in the program book and a picture to be used during the awards presentation.
